Chronic Medical Case Management Services for Children

This is a division of our Children’s Targeted Case Management Services. This service specializes in Chronic Medical case management. As with our Children’s Targeted Case Managers, our children’s Medical Case Managers are able to provide ongoing assessments in an effort to connect families with appropriate resources and programs such as:

  • Assist with Medical and Dental Appointments/Referrals such as:
    • Medication Management
    • Outpatient Therapy
    • In-home Support Services
    • Parenting and Educational Workshops
    • Educational Appointments IEPs (Individual Education Plan) 504s (Medical education plan)
  • Referrals for Sensory Integration Support
  • Assist with financial support
  • Provide Housing Assistance
  • Provide information regarding Respite care
  • Assist with providing Recreational Information (as available)

Children’s Medical Case Managers are also available to provide additional support and care needed specifically geared toward the client’s individual medical needs.

Requirements:

  • Must be under 21 years old
  • Must have Mainecare or Katie Beckett (If assistance is needed to obtain Mainecare or Katie Beckett, please contact the office.)
  • Must have one of the following diagnosis:
    • Autoimmune diseases
    • Diabetes
    • Respiratory conditions/disorder
    • Neurological disorders
    • Brain Injury
    • Physical condition or the present of conditions suggestive of damage to the central nervous system or of later atypical physical development. These would include but are not limited to the following:
      • Cerebral Palsy
      • Meningitis
      • Heart defects
      • Bronchopulmonary dysplasia
      • Or other medically chronic condition recognized by the office of Maine Care
